Matching List of Special School and the District Support Centre for Persons with Disabilities attaching Designated Team for Special School Leavers

Introduction：
In April 2024, SWD has set up a designated team in each of the 21 District Support Centres for Persons with Disabilities (DSCs) in the territories to commence the support service for special school leavers and collaborate with special schools to support the special school leavers and their family members/ carers. For the matching list, please refer to the following table.
