Based on a medical-social collaboration, the Scheme provides appropriate support services for elderly persons with mild or moderate dementia in the community to help stabilize progression of the disease and alleviate their distress of frequenting hospitals, as well as provides related support to carers to achieve "Ageing in Place". The Scheme has covered 41 District Elderly Community Centres and 7 Hospital Authority clusters in the territory. It is estimated that over 2 000 elderly persons with dementia will receive the services each year.
