What is Professional Outreaching Team for Private Residential Care Homes for Persons with Disabilities (POT)?
POT is to provide multi-disciplinary outreach services for residents of private residential care homes for persons with disabilities (including persons with physical and / or intellectual disabilities, persons in mental recovery or persons with suspected mental health problems, etc.) so as to meet their social and rehabilitation needs.
Eligibility for the Service
People with disabilities who are:
- Residents of private residential care homes for persons with disabilities (include those participate in the "Bought Place Scheme for Private Residential Care Homes for Persons with Disabilities") ;
- Family members or carers of residents of private residential care homes for persons with disabilities / Staff of private residential care homes for persons with disabilities.
