大埔及北區福利辦事處本年度推出名為「陪您共渡 照顧者美麗旅程」一系列宣傳及支援活動，為照顧者加強支援，與他們一起同渡照顧旅程。
當中照顧者支援工作小組製作了「照顧者規劃小貼士」，向長者／殘疾人士的照顧者提供參考框架，介紹在安排照顧計劃時需注意及考慮的因素，並因應現有的個人／家庭資源及相關的社區服務，作出照顧計劃選擇，讓照顧旅程中有更好的規劃。
鼓勵各單位於不同群組推廣及宣傳以下電子書連結，一書在手以協助照顧者規劃合適的照顧安排及了解區內服務資訊。
